HOW TO REQUEST A LICENSE ROSTER

Information Included on Rosters

All rosters include the following public information:

  • Licensee Name
  • License Number
  • License Type
  • License Status
  • Issue Date
  • Expiration Date
  • City, State and Zipcode
  • Country

Information Not Included on Rosters

We cannot include the following information on rosters because it is not public information:

  • Mailing Addresses
  • Phone Numbers
  • Email Addresses

Obtaining Disciplinary History

Disciplinary history is not available on rosters at this time. To obtain a licensee's disciplinary history, submit a Written Verification Request , or click Verify License Online.

How Rosters are Sent

We email rosters to you. They are in Excel spreadsheet format. Each roster includes all active licenses under the jurisdiction of the profession(s) you select.

Example : A roster for the Medical profession includes all active

  • Physicians
  • Respiratory Care Practitioners
  • Paramedics

How to Request a License Roster

  • Enclose a check or money order made payable to the "State of Delaware" in the amount of $32.00 for each profession requested.
Note : We cannot accept cash or credit card payment at this time.
