Information About Renewal Applications for All Registrations
Controlled Substance registrations expire on June 30 of odd years (e.g., 2009, 2011).
Several weeks before your registration expiration date, a renewal notice will be mailed to the mailing address on your record. This notice will explain how to access the online renewal application. To complete the online renewal application, you must use a credit card to pay your renewal fee. The notice will also explain how to request a paper renewal application if you are unable to renew online.
Please keep your contact information in our records up-to-date so that notices we send will reach you.
- If the registration is for a practitioner (such as a Physician or Advanced Practice Nurse), you may check and update your contact information online. Click Change Contact Information.
- If the registration is for a facility (such as a Pharmacy or Hospital/Clinic), contact the Division of Professional Regulation to update the facility’s contact information. A facility’s address cannot be updated online.
How to Renew a Lapsed Registration
If you do not renew your registration before the expiration date, the registration will lapse.
- It is illegal for a practitioner to prescribe, store or dispense controlled substances while the registration is lapsed.
- It is illegal for a facility to store or dispense controlled substances while the registration is lapsed .
A lapsed registration may be renewed for a period of one year after the expiration date by submitting a renewal application online and paying a late fee. However, if the registration has been lapsed for more than one year, you must reapply. |
