Sedation or Anesthesia Permits

General Information

For information about Restricted Permits I and II and Unrestricted Permits, refer to Section 7.0 of the Board's Rules and Regulations.

Applying for Permit

Submit:

Completed, signed and notarized Application for Sedation/Anesthesia Permit
Processing fee by check or money order made payable to the "State of Delaware"
Documentation required by the application form.

The Anesthesia Advisory Committee reviews applications under the direction of the Board. The Committee also conducts inspections as applicable before permits are issued.
