APPLYING FOR A NEW LICENSE

Submit a signed and notarized application for examination, along with a pro-rated processing fee (Fee Schedule), to sit for the Practical Examinations in Dentistry or Dental Hygiene. Check or money order must be made payable to the "State of Delaware." An application can be obtained from this web site or by calling the Board office at (302) 744-4533.

Upon successfully passing the Delaware Board examination in dentistry, to be licensed, dentists must provide the following:
  • Proof that you attended a college or university recognized by the Board for at least two (2) years and there taken such courses as the Board shall prescribe.
  • Proof that you graduated from a dental college approved by the Board and that during the four years in dental school has maintained a scholastic standing in all of the prescribed courses of such rank as the Board may from time to time prescribe. A certified transcript must be sent directly from the school.
  • Evidence that you spent one year as a dental intern or resident in a school, institution or hospital approved by the Board. An applicant must submit proof of completion of a general practice residency (GPR), or its equivalency, approved by the American Dental Association (ADA) or has participated in a preceptorship program established under Section 1105 of the dental statute, or has had three years of active dental practice in some other state or territory of the United States. If licensed in another jurisdiction, you must request a self-query from the National Practitioner Data Bank and the results must be provided to the Board. The telephone number is (800) 767-6732.
  • Evidence of completion of the National Board Examination with a score of at least 75 on Part I and Part II of the examination.
  • Proof of current certification in cardiopulmonary resuscitation (CPR) technique.
  • Submit proof of good moral character and reputation.

Upon successfully passing the Delaware Board examination in dental hygiene, to be licensed, dental hygienists must provide the following:

  • Proof of graduation from high school and a dental hygiene program approved by the Board. A certified transcript must be sent directly from the Hygiene program school to the Board.
  • Evidence of completion of the National Dental Hygiene Board Examination at a 75% competency level.
  • Proof of current certification in cardiopulmonary resuscitation (CPR) technique.
  • Submit proof of good character.
How to apply for a permit for sedation or anesthesia?

Submit a signed application for sedation or anesthesia permit, along with a pro-rated processing fee. Check or money order must be made payable to the "State of Delaware." An application can be obtained from this web site or by calling the Board office at (302) 744-4533.
