Submit:

• Completed, signed and notarized application
• Fee by check or money order made payable to " State of Delaware"
• Proof of general liability insurance for at least $1 million
• Proof of errors and omissions insurance for at least $1 million.

In addition, submit the following for inspectors employed by the agency:

• Verification of Employment form completed by employer or, where not obtainable, tax forms W-2 or Schedule C’s
• Proof of passing the Electrical One- and Two-Family Dwellings, Electrical General and Electrical Plan Review examinations.
