Funeral Establishment Permit

HOW TO APPLY FOR A FUNERAL ESTABLISHMENT PERMIT

Please submit the following:

Completed, signed and notarized application form.

Pro-rated processing fee (non-refundable), by check or money order, made payable to "State of Delaware."

Copy of State of Delaware, Division of Revenue, business license for the establishment.

When all of the above requirements have been met and the Board approves issuance of the permit, the Board office will issue the permit.
