These instructions apply if the applicant is filing for licensure as a massage therapist, whether or not the applicant is licensed in another state or jurisdiction.
Requirements for All Applicants
ALL APPLICANTS MUST BE AT LEAST 18 YEARS OF AGE
Please submit the following items to the Board office:
- Completed, signed and notarized Application for
Massage Therapist Licensure form.
Follow the instructions on the application carefully
so that all needed questions are completed. This includes
verification under oath that the applicant has not engaged
in any acts or offenses that would be grounds for disciplinary
action pursuant to 24 Del. C., § 5308
(a)(4).
- Non-refundable processing fee
by check or money order made payable to "State of Delaware."
- Official examination scores sent directly from the National Certification Board for Therapeutic Massage and Bodywork (NCBTMB) to the Board office.
- Copy of current CPR certification card (front and back)
A score report received from the applicant, rather than directly from NCBTMB, is considered unofficial. If the applicant submits an unofficial copy of the score report, the Board will consider the application when the Board office receives all of the other required documents. However, no license will be issued until the Board office receives the official score report from NCBTMB.
For information about the NCBTMB examination, request a candidate handbook from NCBTMB at (800) 296-0664 or visit the NCBTMB web site at http://www.ncbtmb.org/applicants_candidate_handbook.php. The Score Report Request Form is included in the candidate handbook.
If the applicant is already a Certified Massage Technician (issued by the Delaware Board), it is not necessary to re-submit documents already in possession of the Board office. Contact the Board office for more information. See Fee Schedule for fee for upgrading an active Massage Technician license to a Massage Therapy License.
Additional Requirement for Applicants by Direct Application
An applicant is considered to be filing by "direct application" if he or she:
If the applicant is filing by "direct application," he or she must also submit:
- Official transcript sent directly
from the applicant's massage school or approved program
to the Board office. The transcript must show that the
applicant has completed a 500-hour course of supervised
in-class study of massage of no less than 100 hours
of anatomy and physiology, 300 hours of technique and
theory of massage or bodywork therapy, and 100 hours
of elective courses in the field of massage therapy.
Please note: A transcript received from the applicant, rather than directly from the school, is considered unofficial. If an applicant submits an unofficial copy of the transcript, the Board will consider the application when the Board office receives all of the other required documents. However, no license will be issued until the Board office receives the official transcript directly from the school or program.
Additional Requirement for Applicants Certified or Licensed by Another Jurisdiction
If the applicant is now (or has ever been) certified or licensed to practice massage by another state or other jurisdiction, please submit verification of licensure from each state or other jurisdiction in which the applicant has ever been licensed or certified. This requirement applies regardless of whether the applicant is filing by "direct application" or "reciprocity."
To request verification of certification or licensure, the applicant should contact the licensing board for each jurisdiction in which he or she has ever been licensed and ask each to send a verification letter, which is also called a letter of good standing, to the Delaware Board. Contact information for other state Boards is listed on the http://www.amtamassage.org/about/chaplist.html web site. Copies of licenses are not acceptable. For jurisdictions other than states (such as city, county or Canadian province), request the verification from the agency or organization that issued the certificate or license.
Deadline for Submissions
To assure consideration of your license application at the next meeting, the Board office must receive all of these items no later than 4:30 PM ten full working days before the meeting date:
- Completed, signed and notarized application form
- Fee payment, and
- All required supporting documentation (see “License Requirements” on the menu to your left).
Board Review of Applications
If the application for licensure is approved and no other documentation is needed, the Board office will issue the license.
