How long does it take to complete the application process?
Answer:
The application process varies based on how quickly the required forms are submitted
to the Board office. It is suggested that the applicant contact the Board office on a
monthly basis to check on the status of his/her application. Once the application is
complete, it will be listed on the next month's Board meeting agenda. If approved for
licensure, the applicant is advised in writing of the pro-rated license fee. When the
Board office receives the license fee, a license is issued.

Answer:
The licensee/former licensee may do either of the following:
- Submit a written request to the Delaware Board advising that a written license verification to be sent to the state Board where he/she is applying for licensure. This request must be submitted with a $10 check or money order made payable to the State of Delaware; or
- Submit to the Delaware Board the other state licensing Board's prescribed verification form. This form will require the licensee/former license to complete a portion of that state's form. The form must be submitted with a $10 check or money order made payable to the "State of Delaware".

and be submitted with a copy of a marriage certificate,
divorce decree or other legal document and the licensee's
professional license number. Name changes may be faxed
or mailed to the Board office.
