How long does it take to complete the application process?

Answer: The application process varies based on how quickly the Board office receives the required documents and whether the application is for reinstatement of a lapsed license. When the application that is not a reinstatement is complete, it will be reviewed and, if approved, the license will be issued. Applications for reinstatement will be listed on the agenda for the Board’s next meeting. If the Board approves the reinstatement, the license will be issued.

 

What is the process and cost for a written license verification to be sent to another state?

How do I change my name or address?

Answer: For security reasons, you must submit name change requests in writing and enclose a copy of the legal document showing the name change (e.g., marriage certificate, divorce decree). See the Request for Name Change form. You may submit address changes online. See Change Contact Information.

 

How do I apply to become an approved continuing education provider in Delaware?

Answer: File the Request For Pre-Approval of Continuing Education form following the instructions on the form.
