HOW TO APPLY FOR A NEW LICENSE BY EXAMINATION

An applicant who is applying for licensure as an optometrist must meet the following requirements:

  1. Receive a degree of "Doctor of Optometry" from a legally incorporated and accredited optometric college or school, which has been approved by the appropriate accrediting body of the American Optometric Association. Official transcript showing earned graduate degree sent directly to the Board office.
  2. Achieve the passing score on the substantiated clinical portion of the national board examination given by the National Board of Examination in Optometry. The clinical examination may be taken as part of the National Board Examination.   3. Complete a 6-month internship in optometry, which must be approved by the Board.
  4. Has not engaged in any of the acts or offenses that would be grounds for disciplinary action and has no disciplinary proceedings or unresolved complaints pending against him or her in any jurisdiction where the applicant has previously been or is currently licensed as an optometrist.
  5. Possesses a current cardio-pulmonary resuscitation (CPR) certification for adult and children.
To process your application, the Delaware Board of Examiners in Optometry requires:

  • Completed application form, which has been signed and notarized. The application is available on this website.
  • Non-refundable pro-rated fee in the form of a check or money order made payable to the "State of Delaware".
  • Transcript from college of optometry.
  • Copy of Optometry National Board examination scores, Part I - III and TMOD.
  • Up-to-date certification to perform CPR for adults and children.
  • Statement of Supervising Doctor completed by each doctor who will supervise you during your internship.
  • Report from the Healthcare Integrity Protection Databank (see below).

Applicants for a Delaware Therapeutic Optometrist license must submit a report from the Healthcare Integrity and Protection Data Bank. The report provides history of disciplinary actions, medical malpractice, changes in privileges and other information. To obtain a self-query application, you may call the Data Bank at 800-767-6732 or access the Data Bank website. The Data Bank will send the report to you. You must then submit the original report to the Board office.
