License Renewal



Renewal Information

Licenses expire on June of odd years (e.g., 2007, 2009).

Effective with your 2007 renewal, the Division of Professional Regulation offers online renewal with the use of a major credit card. You will receive a notice to advise you when your renewal application is available for submission online. This will be your only renewal notice. The notice will explain how to access online renewal and how to request a paper renewal application if you are unable to renew online.

You will be asked to attest to completing the required continuing education (CE) in accordance with Section 10.0 of the Board's Rules and Regulations. If you are licensed as a Therapeutic Optometrist, you will also be asked to attest to maintaining current cardiopulmonary resuscitation (CPR) certification for adults and children. Please note that CE reporting forms and copies of your CPR card are no longer required. However, a percentage of licensees will be randomly selected for audit of their CE and CPR certification following the renewal.

Note: Please do not send any CE reporting forms, CPR cards or other documentation to the Board office unless you are requested to do so.

How to Renew a Lapsed License

If you do not renew your license before the expiration date, your license will lapse. It is illegal to practice optometry while your license is lapsed.

You may renew a license for a period of one year after the expiration date by submitting a renewal application, paying a late fee and attesting to completion of the required CE. The renewal application will be available online. However, if the license has been lapsed for more than one year, you must reapply.
