New Pharmacist License


PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY BEFORE SUBMITTING THE APPLICATION

NOTE: Answer all questions completely. The application must be legible.

How to apply for a Pharmacist license:

Graduating Pharmacist

Applicants must:

  • Be a registered Delaware intern.
  • Submit a completed and notarized Application for Examination as Pharmacist
  • Submit a non-refundable, pro-rated fee. Please refer to the Fee Schedule for the appropriate fee. Make the check or money order payable to the “State of Delaware”.
  • Pass the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE). Register for the exams at www.nabp.net. For MPJE study material, click here.
  • Submit 1500 hours of Board-approved internship.

Reciprocating/Transferring Pharmacist

Applicants must:

  • Complete the NABP Preliminary Application for Transfer of Pharmaceutic Licensure located at www.nabp.net and send it to NABP with your check or money order.
  • Mail your Official Application for Transfer of Pharmaceutic Licensure and non-refundable prorated fee to the Delaware Board of Pharmacy. Please refer to the Fee Schedule for the appropriate fee. Make the check or money order payable to the “State of Delaware”.
  • Pass the Multistate Pharmacy Jurisprudence Examination (MPJE). Register to take the MPJE at www.nabp.net. For MPJE study reference material, click here.

Foreign Pharmacist

Applicants must:

  • Submit a completed and notarized Application for Registration of Internship (Foreign Graduate).
  • Submit a copy of the Foreign Pharmacy Graduate Examination Committee (FPGEC) Certificate. For an application, visit www.nabp.net.
  • Earn 1500 internship hours of Board-approved internship from the date of FPGEC Certification.
  • Submit a non-refundable fee for the intern and pharmacist applications. Please refer to the Fee Schedule for the appropriate fees. Make the check or money order payable to the “State of Delaware”.
  • Submit a completed and notarized Application for Examination as Pharmacist.
  • Pass the North American Pharmacist Licensure Examination (NAPLEX) after obtaining your FPGEC Certificate and pass the Multistate Pharmacy Jurisprudence Examination (MPJE). Register to take the exams at www.nabp.net. For MPJE study material, click here.
