Information About Renewal Applications for All Licenses

Pharmacist licenses and Pharmacy permits expire on September 30 of even years (e.g., 2010, 2012).

Several weeks before your license expiration date, a renewal notice will be mailed to the address on your record. This notice will explain how to access the online renewal application. To complete the online renewal application, you must use a credit card to pay your renewal fee.

Information About Renewal Applications for Pharmacist

You will need to provide information about continuing education (CE). The CE rules are in Sections 1.3 and 1.4 of the Board’s Rules and Regulations.

Information About Renewal Applications for Pharmacies

You will need to provide information about the pharmacy’s mailing and location addresses, Pharmacist-in-Charge (Retail and Hospital pharmacies), and ownership.

How to Request Inactive Status and Reactivate Pharmacist Licenses

If you are not actively practicing, you may submit a written request to place your license on inactive status to the Board Office.

How to Renew a Lapsed License

If you do not renew your license before the expiration date, your license will expire. It is illegal for a Pharmacist to practice or a Pharmacy to operate while your license is lapsed.

You may renew a lapsed license for a period of 60 days after the expiration date by submitting a renewal application online and paying a late fee. However, if the license has been lapsed for more than 60 days, you must reapply.
