Reciprocity Licensure - Physical Therapist/Physical Therapist Assistant

HOW TO APPLY FOR A PHYSICAL THERAPIST OR PHYSICAL THERAPIST ASSISTANT LICENSE BY RECIPROCITY
These instructions apply if the applicant is filing for a license as a physical therapist or physical therapist assistant and he or she already holds a current license in another state. If the applicant has already passed the National Physical Therapist Examination but does not hold a current license in another state, please see the instructions for applying for an original license.
Requirements for All Applicants
Please submit the following items directly to the Delaware
Examining Board of Physical Therapists and Athletic Trainers
(The Board") office at the following

- Submit an official transcript sent directly
from the college or university to the Board office if all of the following
conditions are met:
- The applicant has already graduated, and
- The applicants's physical therapy degree is from a school accredited
by Commission on Accreditation of Physical Therapy Education (CAPTE),
and
- The physical therapy degree is not a transitional Doctorate of Physical Therapy degree from a U.S. school, see Additional Requirements for Foreign-Trained Applicants below.
- Verification of licensure from each state in which the applicant has ever been licensed, and
- Your Scores on the National Physical Therapist Examination, sent directly from the Federation of State Boards of Physical Therapy (FSBPT) to the Board office.
To request your scores, complete a Score Transfer Request Application. This form is available at
https://www.fsbpt.net/pt.
To request verification of licensure, send a request to each state in which you have ever been licensed. Each state should return the verification directly to the Board office. Note that each state may charge a fee for this service.
Additional Requirements for Foreign-Trained Applicants
If the applicant received his or her education outside
the U.S. at a school that is not accredited by the Commission
on Accreditation of Physical Therapy Education (CAPTE),
the applicant must submit a credential evaluation in addition to the
documents listed above. This requirement applies even if the applicant
has received a transitional
Doctorate of Physical Therapy for a U.S. school. The
evaluation must be prepared by an agency approved by the Board. The Board currently
accepts evaluations from:

The credential evaluation should be sent directly to the Board office.
Information About Temporary Licenses
An applicant may apply for a temporary license to work in Delaware while awaiting permanent licensure. A temporary license is issued after verification letters are received from each state in which the applicant has been licensed and generally remains in effect for a period of up to 90 days. While under temporary licensure, the applicant must practice under the direct supervision of a Delaware-licensed physical therapist. In addition to all of the requirements above, the applicant's supervising physical therapist must complete a Statement of Supervising Physical Therapist or Athletic Trainer and send it to the Board office.
When the temporary license is approved, the Board office
will notify the applicant to submit the temporary license
fee payable to the "State of Delaware". This fee is in
addition to the pro-rated
processing fee above and is non-refundable. The applicant should not send
this fee until notification is received from the Board
that the temporary license has been approved.
Notification of Examination Results
If the application for licensure is approved, the Board office will issue the license.
