Reciprocity Licensure - Athletic Trainer


HOW TO APPLY FOR AN ATHLETIC TRAINER LICENSE BY RECIPROCITY

These instructions apply if the applicant is filing for a license as an athletic trainer and already holds a current license in another state. If the applicant has already passed the National Athletic Trainers Association (NATA) examination but does not hold a current license in another state, see the instructions for applying for an original athletic trainer license.

Requirements for All Applicants

Please submit:

  • Completed, signed and notarized application form (please answer all questions)
  • If the applicant has already graduated, an official transcript showing degree conferred and dates sent directly from the school to the Board office.
  • If the applicant has not yet graduated, letter from school official stating that the applicant has completed graduation requirements and the expected date of graduation.
  • Verification letter from National Athletic Trainers Association Board of Certification (NATABOC) sent directly to the Board office.
  • Verification letters from all states in which the applicant has ever been licensed.

If the applicant submits a letter from a school official, the applicant must also submit the official transcript showing the degree conferred and dates as soon as it is available. No permanent license will be granted until the transcript is submitted to the Board office.

Information About Temporary Licenses

An applicant may apply for a temporary license to work in Delaware while awaiting permanent licensure. A temporary license generally remains in effect for a period of up to 90 days. While under temporary licensure in a clinical setting, the applicant must practice under the direct supervision of a Delaware-licensed PT. While under temporary licensure in a non-clinical setting, the applicant must practice under the direct supervision of a Delaware-licensed AT. To apply for a temporary license, the applicant's supervising PT or AT, whichever applies, must complete a Statement of Supervising Physical Therapist or Athletic Trainer and send it to the Board office.

When the temporary license is approved, the Board office will notify the applicant to submit the temporary license fee payable to the "State of Delaware". This fee is in addition to the pro-rated processing fee above and is non-refundable. The applicant should not send this fee until notified that the Board office has approved the temporary license.

Notification of Licensure

If the application for licensure is approved, the Board office will issue the license.
