Licensure Requirements


PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY BEFORE SUBMITTING THE APPLICATION AND NON-REFUNDABLE APPLICATION FEE

  • NOTE: The application must be typed or printed legibly. Answer all questions completely. Be certain that all signatures are placed where they are required. Incomplete applications will be returned to the applicant. The Board will not review an application for licensure unless the Board office has received all supporting documentation.
The following is required of all applicants:
  • Submit an official National Practitioners Data Bank Report. The applicant may obtain the required form by contacting the National Practitioners Data Bank at the following address:

Licensure procedure for direct licensure:

  • Applicant must hold a degree or Doctor of Podiaric Medicine from a college or school approved by the Council on Podiatric Education or the American Podiatric Medical Association or its successor.
  • Submit a completed notarized application. You may download the application from our website.
  • Submit an affidavit that the applicant has not engaged in any acts or offenses that would be grounds for disciplinary action pursuant to 24 Del. C., Chapter 5, § 508 (a)(4) through (8).
  • Submit the appropriate pro-rated licensure fee in the form of a check or money order made payable to the "State of Delaware". This fee is non-refundable.
  • Request college to send your certified transcript directly to the Board office.
  • Submit verification of having passed the National Board of Podiatric Medical Examiners (NBPME) Examinations, Parts I and II. Official score report must be sent from the exam service directly to the Board office.
  • Submit official verification of having satisfactorily completed a one year hospital residency program, approved by the American Podiatric Medical Association, or a one year preceptorship in the office of an approved licensed podiatrist in the State of Delaware.
  • Applicant's must take and pass the Podiatric Medical Licensing Examination for States (PMLEXIS) Level 3.
  • Request National Practitioner Data Bank to send verification of applicant's licensure status directly to the Board office.
The following is required if applying for a temporary license:
  • Applicants who meet the educational and residency requirements and who will be taking the next scheduled PMLEXIS may be granted a temporary license for an interim period. The temporary license may be renewed only one time. If the applicant does not pass the examination, the temporary license will expire immediately.
The following is required for those applying for direct licensure that have no residency background:

  • Applicants who have not completed a hospital residency program may seek a one-year preceptorship in Delaware. The objective of the preceptorship is to serve for a twelve (12) month period, in the office of the primary sponsor, on an active basis, for a minimum of twenty (20) hours per week for fifty (50) weeks. The perceptorship shall pattern itself after and ensure exposure to all areas covered by the CPME for Podiatry of Podiatric Residence Section 21.0 through 21.6.
  • Professional liability insurance coverage will be provided by the preceptor for the candidates at all times during the perceptorship. State Board of Podiatry approval is required before the program begins.
  • Once the perceptor has been obtained by the applicant and approved by the Board, he/she will assist the applicant in contacting a potential secondary sponsor. The perceptor is the primary clinical observer. The perceptor and the secondary clinical observer must keep accurate records of the applicant's performance in a variety of categories. The records will be evaluated by the Board of Podiatry at the conclusion/termination of the preceptorship.
