License by Direct Application


Apply by Direct Application if either of these situations applies to you:

  • You recently completed your residency year and are not licensed in another state or jurisdiction.
  • You are licensed in a state or jurisdiction where the licensure requirements are not substantially similar to those of Delaware and you do not have at least five years practice in podiatric medicine.

Note: To find out if the licensure requirements of your state or jurisdiction are substantially similar to Delaware’s, see Section 4.2 of the Board’s Rules and Regulations.

If you hold a current license in another state or jurisdiction or you have at least five years of practice, see Reciprocity.

How to Apply

Submit:

  • Completed, signed and notarized Application for Licensure to Practice Podiatric Medicine
  • Processing fee by check or money order made payable to “State of Delaware”
  • Official report from the National Practitioners Data Bank (NPDB). To request the report, follow the self-query instructions on the NPDB website.
  • Official transcript from a college or school approved by the Council on Podiatric Education or the American Podiatric Medical Association sent directly from the school to the Board office. The transcript must show that you have a DPM degree or its equivalent.
    • If you graduated from a school outside the U.S., arrange for a credentialing agency to evaluate your transcript and submit the evaluation to the Board office.
  • Official score reports for these examinations, sent directly to the Board office:
  • Proof of completion of a one-year hospital residency program approved by the American Podiatric Medical Association or a one-year preceptorship in the office of an approved, licensed podiatrist in the State of Delaware.
  • Verification of licensure from each state in which you currently hold, or have ever held, a license to practice podiatry, sent directly from each State Board to the Board office
