Registered Vendors
Delaware law requires all prescribers of any controlled or non-controlled substance to obtain and use tamper-resistant prescription forms ordered only from vendors approved by the Division of Professional Regulation (DPR).
DPR considers manufacturers, distributors and printers of tamper-resistant prescription forms to be vendors. Any manufacturer, distributor or printer seeking DPR approval as a registered vendor of tamper-resistant prescription forms must apply to DPR using the Application for Registration as a Vendor of Tamper-Resistant Prescription Forms.
The following lists are the registered vendors DPR has approved as of the date of this web page. DPR will update these lists as additional vendors are approved.
Information for Prescribers: DPR-Approved Distributors and Printers
The following DPR-approved vendors accept orders directly from prescribers. Prescribers should select a vendor from the list below and contact the company directly to place an order for tamper-resistant prescription forms. Prices may vary. Prescribers will need to have their DPR-issued Security Code each time they place an order.
