How to apply for a license by reciprocity

These instructions apply if the applicant is filing for licensure as a Licensed Chemical Dependency Professional and he or she already holds a current license in another state. Applicants must provide the following:

  • Signed and notarized application, along with the appropriate pro-rated fee. Check or money order must be made payable to the "State of Delaware." An application can be obtained from this web site or by contacting the Board office.
  • Verification of licensure in good standing from the other state(s) and an Affidavit and Release of Information sent directly from the other states to the Board office.
  • If applicant has practiced in good standing for five (5) years in the other jurisdiction, no further documentation is needed. However, if the applicant has practiced less than five (5) years, he or she must submit a copy of the other state's statute/rules and regulations to determine whether that state's laws are substantially similar to those of Delaware.
