How to Apply for a New (or Initial) LPCMH License

To apply for the Licensed Professional Counselor of Mental Health (LPCMH) by certification, submit the following:

  • Completed, signed and notarized Professional Counselor Application including the Notarized Affidavit affirming you meet the qualifications as stated in 24 Delaware Code, Chapter 30, Section 3032(a) (1) through (6).
  • Processing fee by check or money order made payable to the "State of Delaware."
  • Official transcript showing completed graduate degree, sent directly to the Board office from the college or university.
  • Completed Direct Supervision Reference and Professional Counseling Experience Reference forms showing professional counseling experience and professional direct supervision. Professional counseling experience must consist of not less than 3,200 hours over a period of not more than four (4) years, at least 1,600 hours of which must have been under professional direct supervision acceptable to the Board. The experience must be obtained in a clinical mental health setting acceptable to the Board.
  • Verification of certification from the National Board of Certified Counselors (NBCC), the Academy of Clinical Mental Health Counselors (ACMHC), or other national mental health specialty acceptable to the Board.

How to Apply for a New (or Initial) LACMH License

To apply for the Licensed Associate Counselor of Mental Health (LACMH), submit the following:

  • Completed, signed and notarized Licensed Associate Counselor Application
  • Processing fee by check or money order made payable to the "State of Delaware."
  • Official transcript showing completed graduate degree, sent directly to the Board office from the college or university.
  • Verification of certification from the National Board of Certified Counselors (NBCC), the Academy of Clinical Mental Health Counselors (ACMHC), or other national mental health specialty acceptable to the Board.
