Fee Schedule

Processing Fees for License Applications

Submit the processing fee for the type of license application you are filing with the application form. Applications submitted without the processing fee will be rejected. Make the check or money order payable to the “State of Delaware.” Cash is not accepted. Processing fees are non-refundable.


Athletic Trainer – $158

Athletic Trainer Reinstatement – $237

Physical Therapist Assistant – $148

Physical Therapist Assistant Reinstatement – $222

Physical Therapist – $158

Physical Therapist Reinstatement – $237

Temporary (All Types) – $40

Upgrade from Active PTA to PT – $55


Other Fees

Continuing Education Application – $75

Paper Examination Request – $200

Renewal Fee – You are notified of the amount of the renewal fee at the time of renewal. All renewals, including renewals of inactive licenses, require a renewal fee. When you submit a renewal application after the expiration date, you must pay a late fee in addition to the renewal fee. The late fee is 50% of the renewal fee.
